image not showing, replace command not showing

i have a new template with a few editable images in it.

one of them is not working:  when the template content is edited, all the other images show the "replace" link, but not this one. It just shows a white box, around 150x150.
I can click on the white box and it asks if i want to delete the image, or browse to a new image.
So it thinks it's there.
But after browsing to an image and saving it, the white box returns.
If I preview the email, there is no image there - just the alt text.
if i send the email, there is no image.

this is the code:
<td align="left" valign="top" class="signaturebar"><img src="<$imagesrc link='false'$>" alt="signature"></td>

my css just has:
      td.signaturebar {         padding: 5px 20px;    }

looked at the help at http://webqem0.createsend.com/help/topic.aspx?t=130
there are no spaces in the filename. It's an unremarkable jpg.

any ideas?
gotta send this email tomorrow!
