improved signup form markup

Hi guys.

Just wanted to post an alternative version of the signup form markup which I think is a little more sematically correct than the one provided within CM.

Change values as you see fit ;)

I have also used the CM AJAX subscription form and taken it a little further here.

You'll need the relevant files from the CM AJAX subscription form.

I'll package something up for everyone when I get a chance.

Our default signup forms were create a while back, and could certainly be improved upon, we appreciate you providing this for people.
