Bullets don't show in Outlook 2010 and "back to top" doesn't work :-(

Hi All
Close to final mail out and now I've discovered that my bullets in my
don't show in Outlook 2010 they do in Outlook 2007 and Entourage.

I also have "back to top" links which while they work in the browser version and Entourage, don't work in Outlook 2010 or 2007. Ideally I'd like these to link to just under the welcome letter which is below my banner, where the contents list is.

Does anyone have any ideas please?

Here's my code:

Welcome to the first issue of SPHERE Connect, the official SPHERE newsletter that will inform you of developments in HER2 testing, update you on the SPHERE programme’s progress and keep you in contact with your colleagues throughout the SPHERE region.
It has been several months since we first met under the SPHERE banner, and I am happy to report that the SPHERE roll-out meetings are going from strength to strength. The national phases of the training programme have been launched and have reached our colleagues across the Asia Pacific region in China, South Korea, Thailand, Malaysia, Indonesia, Vietnam, the Philippines, Hong Kong and Taiwan.
The programme itself continues to evolve. Within this first edition of SPHERE Connect, we introduce the SPHERE Clinical Audit. This will produce the region’s first HER2 testing database in breast and gastric cancer which will be of great assistance in achieving best practice in HER2 testing. Needless to say, SPHERE participants will figure prominently in this project.
This newsletter also features updates from the recent American Society of Clinical Oncology (ASCO®) meeting in Chicago, alongside developments across the region.
SPHERE Connect is very much your platform to connect, share and debate with colleagues. We welcome your contributions; your experiences, cases and especially your feedback. For more details on how to submit your ideas, see the section “SPHERE Connect is your voice! . We look forward to hearing from you and hope that SPHERE Connect will prove to be a valuable resource.
Variations in HER2 test results not only reflect technical problems, but also the imperfect assumptions that currently guide testing guidelines and scoring criteria, as highlighted Professor Giuseppe Viale of the European Institute of Oncology, University of Milan.
Professor Viale emphasised the necessity for greater collaboration between oncologists and pathologists to address problematic cases that affect prescriptions of HER2-targeted treatments. His review of the scoring process raised several uncertainties related to diagnostic thresholds for determining HER2 positivity, reclassification of HER2 positivity status in metastatic tumours, identification of genuine polysomy cases, suitability of HER2/CEP17 as a scoring parameter and HER2 classification in intratumoural heterogeneity.
He provided several suggestions to address these uncertainties.

When HER2 status changes, retest the primary tumour and metastases simultaneously to rule out technical variations, preferably with alternative assays.

If overexpression is present in less than 10% of cells, assess HER2 status in all blocks of the primary tumour and report actual percentages as the patient may benefit from re-biopsy.

The HER2 gene count number is more appropriate than the HER2/CEP17 ratio for establishing amplification in polysomic cases.

For intratumoural heterogeneity, double staining of the same specimen allows identification of individual tumour cells with HER2 gene amplification and overexpression.

D. Johnston of the Royal Marsden Hospital, London.
The MAP 3 trial results were among the more significant developments in breast cancer prevention among high-risk patients. These results demonstrated a reduction in annual cancer incidence from 0.55% with placebo to 0.19% with exemestane, an aromatase inhibitor. This translates into a 65% risk reduction. Furthermore, treatment with exemestane was well-tolerated, which may significantly benefit patients who face rare but serious side effects with the current gold standard, tamoxifen.1
Dual targeting of HER2 positive breast cancers in the neoadjuvant setting also emerged as a promising treatment strategy. The combination of lapatinib and trastuzumab with antracycline- or taxane-based chemotherapy produced higher pathological complete remission rates compared with single agent combinations with chemotherapy.2,3 The TBCRC 006 trial demonstrated the efficacy of dual targeting in the absence of chemotherapy, with the addition of endocrine therapy in hormone-positive patients. However, treatment duration must be extended due to the slow development of hormone-positive tumours.4
Dr Johnston concluded that emerging evidence suggested that cell proliferation rates may be able to predict prognosis and determine responsiveness to treatment, particularly in hormone-positive breast cancer.

It is finally here; enrolment for the SPHERE Clinical Audit that will build the first regional HER2 testing database for breast and gastric cancer begins this month. Data compiled will provide insights into centre-, country- and region-specific positivity rates; detect trends in the number of HER2 tests performed and establish the epidemiology of HER2 positive cancers.
The audit encourages best practice and will give you the opportunity to measure your laboratory’s HER2 testing performance, gain expert advice to troubleshoot testing variations, participate in publication opportunities and establish networks with peers and key opinion leaders.
The pilot phase of the audit will run from August 2011 to February 2012; all SPHERE-trained centres across the Asia Pacific region are invited to participate. Before enrolling in the audit, do take the time to understand the protocol thoroughly and clarify:

Validate – Validation of materials and methods builds reliability and reproducibility into testing procedures. Ensure that every phase of the HER2 testing protocol for both breast and gastric cancer specimens are validated, documented and adhered to in the final protocols. Changes to the testing method or reagents, however minor, can introduce variations and must be revalidated.

Standardise documentation – Document each testing phase, along with the final results of each test performed, in standardised log books and forms. This improves traceability and the detection of sources of variation.

Hi AliGee, good to see you again. With bullet links, try using:

.sidebar ul { margin-left: 1.5em; }

Bulleted lists tend to get truncated in Outlook and just need to have margin-left defined.

Secondly, anchor links can be quite problematic in email. You'll see in that linked post that "linked element(s) display ~ 100px below the top of the preview pane", which is something we can't get around on our end, I'm sorry to say.

Let us know if you have any other questions - good luck with your campaign!
